Overview
Custom Forms is a feature within PCR-360's Lighthouse module that allows authorized users to design, publish, and manage custom data-collection forms. Forms can be used internally within Lighthouse or made available to end users through the CustomerCenter (CC) portal.
Permission Required: Users must have the Form Builder permission to create and manage forms. End users only need access to the menu link or CustomerCenter to fill out published forms.
Where to Find It
In PCR-360, navigate to Lighthouse → Form Builder → Manage Forms in the left-side navigation menu. This is the central location for creating and managing all custom forms.
Once a form is created and published, administrators can add a direct link to any Lighthouse menu folder using Settings → Add Form to Menu from within the form editor. A link to the form's submissions grid can also be added via Settings → Add Submissions to Menu.
Use Cases

Internal Lighthouse forms — Collect structured data from PCR-360 users (e.g., requests, surveys, intake forms). Submissions are stored and viewable in a generated Ad Hoc grid.

CustomerCenter forms — Expose forms to CC-only users via the CC portal. When configured with Create CC Request on Submit, a Service Desk request is automatically created and linked to each form submission.
